So, Lex gets shot in the face in Detroit, and Lionel - with access to the BEST doctors and facilities in the world - sends him to Smallville, Kansas for treatment??

I'm not quite sure how long it takes to fly from Detroit to Kansas, but I'm guessing there is a closer and better hospital than Smallville Medical Center.

Then - with Lex's life in eminent danger, how did Lionel get Lex back out of the hospital? I would have liked to hear that conversation:
Lionel: Yes, doctor I know that my son is in the process of dying, but I'm going to take him home now to perform my own highly unethical and illegal experiments on him.
Doctor: Oh, okay. Here's your release form. You can take him whenever your ready..

I thought both of these things weird too. I just chalked it up to some poor writing and tried to look past it.

You bring up a good point! My mom & dad noticed that, too, but for some reason I didn't even think about it as I was watching last night. It could've made sense to lifeflight him to Metropolis, although there are good hospitals closer to Detroit than that, but you wouldn't send anyone to the hospital in Smallville unless they were already there. Why did Kara end up in Detroit, anyway? At least it wasn't New Jersey...

Personally, I don't think it made much sense to have Metropolis located in Kansas in the first place. They should've had it in Illinois, because then it could be on Lake Michigan & have a pier, it still wouldn't be all that far from Smallville (9 or 10 hours isn't much more than 6 hours), & there really is a Metropolis, IL!
